Everest Recovery will screen individuals for the optimal level of care based on the American Society of Addiction Medicine Patient Placement Criteria (ASAM-PPC), ,and will identify any coexisting conditions that indicate the need for additional external professional assessment and/or referral for services.maintain administrative and assessment procedures for the admission, intake, and program orientation.
INTENT:
To ensure that the individual is appropriate for further clinical assessment prior to medical evaluation and subsequent admission.
WRITTEN PLAN:
Everest Recovery will screen each prospective patient to determine his/her treatment eligibility and appropriateness based on the ASAM-PPC Dimensions 1-6.
Since screening is the first contact with a potential patient, Federal regulations regarding confidentiality will apply from the initial contact.
All completed screenings will be entered into the electronic health record.
Screening information shall include:
- Demographics
- Presenting problem(s), including brief descriptions of substance use history, current use and treatment history
- Special needs such as:
- Pregnancy
- Current suicidal/homicidal ideations
- Major psychiatric illness.
- Current medical concerns/problems
The completed patient screening information shall be reviewed by an appropriately credentialed Everest Recovery associate.
DH policy requires that all potential patients be afforded an opportunity to have a face to face medical and clinical assessment.
If screening reveals that the individual is appropriate for further evaluation, an appointment for an intake assessment is given, ideally within 24 hours of the initial contact.
If the clinic is at capacity the patient shall be placed on a waiting list if they desire.
The patient shall be informed of the need to bring the following to the intake appointment to verify their identity, eligibility, and to be able to be medicated:
- State approved picture ID
- Social Security card or birth certificate (which ever is required by State Regulations)
- Proof of income (W-2, check stub, letter from employer) or proof of insurance
- Initial one week fee or Medicaid card
If it is determined that a potential patient admission is not appropriate, Everest Recovery shall provide the patient with referrals to more appropriate treatment facilities.
